Medical Card Consultation & Pricing
At Heal Balanced Medicine, we offer streamlined evaluations to help you secure your Oklahoma medical card. Please bring your valid State ID and $75 cash for the physician’s recommendation and digital upload service. If you are a Medicare or SoonerCare recipient, ensure you bring proof of enrollment to qualify for the state's discounted registration rate.
The state of Oklahoma requires a separate licensing fee paid via credit or debit card during your appointment: $104.30 for standard applications or a reduced $22.50 for those with Medicare or SoonerCare. Our team will guide you through the entire registration process to ensure your application is submitted correctly before you leave the office.
